Participant Release Waiver and Parental Consent Form:
THIS FORM MUST BE READ and SIGNED BEFORE REGISTRATION CAN BE PROCESSED AND CONFIRMED, at the latest, two days in advance of the drama camp session or drama class session start date.
TO WHOM IT MAY CONCERN:
The undersigned does hereby give permission for our (my) child to attend and participate in all activities sponsored by I Heart Drama for the sessions registered.
I hereby voluntarily release, forever discharge Dina Dametto, I Heart Drama, its officers, directors, employees, volunteers and agents from any and all claims, demands, or causes of action, which are connected with my child's participation in the I Heart Drama program.
If for any reason my child requires medical attention beyond that furnished by Dina Dametto and I Heart Drama, I agree to be responsible for any expense incurred. I covenant and agree to indemnify Dina Dametto, I Heart Drama and its employees, directors and staff and save them harmless from any and all lawsuits, actions and prosecutions by reason of any activity carried out by my child, whether on or off the premises at which the camps are held. I authorize Dina Dametto, I Heart Drama to administer first aid to my child and to secure medical care for my child in an emergency as deemed appropriate by the attending physician(s).
I understand that Dina Dametto, I Heart Drama is not responsible for loss of personal articles, or damage of or wear to such articles, that may occur while attending the program.
I expressly release Dina Dametto, I Heart Drama, its officers, directors, agents, employees, licensees and assigns from and against any and all claims for invasion of privacy, defamation, infringement of copyright or any other cause of action that may arise out of such use. I hereby irrevocably release Dina Dametto, I Heart Drama from any and all claims for libel and invasion of privacy in connection with the foregoing.
I agree that Dina Dametto, I Heart Drama and its representatives are permitted to send me e-mails/text messages with respect to schedule and location changes; class and camp scheduling, reminders; notifications regarding new programs, discounts and special promotions; audition opportunities; participation in upcoming productions; early registration discounts; and the like.
I certify that the information provided in this registration form is, to my knowledge, true and complete. I have read and understood the above waiver and consent and confirm that I am the parent or legal guardian of the child mentioned herein who is a minor. I confirm that I have the complete custody, care and control of the minor and have the legal authority to agree to this consent and waiver on behalf of the minor and that the consent of no other person or entity is required.
I agree to pick up the child registered within this online registration (Drama Student) at the scheduled times. I understand that drama students are not permitted to bring or consume food of any kind during drama class time, however they are permitted to bring a water bottle. I understand that registration confirmations and drama class information will be sent by e-mail wherever possible.
Please note: Dina Dametto, I Heart Drama reserves the right to request a withdrawal of a student from the program at her own discretion, where it is deemed the needs of the child cannot be accommodated without unacceptable disruption to the class. A refund may be issued in these cases for drama class days not attended.
Please ensure that Dina Dametto, I Heart Drama are notified well in advance of any medical issues and other related conditions that might present an issue with respect to the ability to accommodate a particular student.
E-mail & SMS Communications: We may need to communicate with parents/guardians and we send these communications via e-mail. Please be aware that these will often go to Spam and Junk mail folders. In order to be sure of receiving communications mark received e-mails from us as non-Spam when you find them there. Alternatively, you can add us to your Safe Senders list, if your provider allows for this. Because of the unreliability of e-mail, we will also, at times, use SMS text messaging to communicate information about schedule and location changes, etc. In compliance with Canadian anti-spam legislation, if you prefer not to receive these emails post camp completion, please notify us via email with the subject line "unsubscribe".
Payment: Payment is required in full using a valid credit card.
Refund policy: Refunds must be requested at least three weeks before the start of drama sessions/camp. Three weeks or more prior to class start: full refund, less $ 50 (for registration processing and fees); 2 weeks up to 3 weeks prior to class start: 50% refund. Less than 2 weeks prior to class start: no refund.
